First 12 Pages are Missing!

I have recently uploaded 3 Kindle version books. Two came out fine but the third has the cover followed by the next page, followed by 12 missing pages and picking up fine after that for the rest of the book. I had a friend order the whole book to verify this problem. thinking the problem might have been in the original upload, I republished. After waiting a couple days for the book to get Live I had another friend order the book and it has the exact same problem the 12 pages following the cover and first page are missing.

This book was created an uploaded from Word as were the other two books that appear fine.

The book in question is Hot Springs of Western Washington.
